The report, published today (Tuesday), stated that at least two of the murders could have been avoided if the hospital management had heeded doctors' warnings and acted sooner. Judge Kathryn Thirlwall, who headed the inquiry, sharply criticized hospital executives and determined that poor management and a lack of understanding of the duty to protect patients allowed Letby to continue working in the department even after suspicion arose that she was harming babies. "Once suspicion arose that Letby might intentionally cause harm, steps should have been taken to protect patients," Thirlwall stated, adding that suspicion alone was enough to remove her from the department. Letby was convicted three years ago of murdering seven babies and attempting to murder seven others, using methods such as injecting air into veins, insulin poisoning, and overfeeding. She was sentenced to life imprisonment without the possibility of release and continues to maintain her innocence. The report includes 17 recommendations for changes in the healthcare system, including placing cameras in cribs and incubators in neonatal units, tightening security on insulin storage, and formulating a national procedure for dealing with suspicion that a medical staff member is harming patients. The British government apologized following the publication of the findings.
