According to a report in Maariv, the Bundeswehr estimates that in the event of a large-scale conflict, up to 1,000 wounded soldiers will arrive daily for treatment in the country's hospitals. Currently, only five military hospitals operate in Germany, which will quickly reach their capacity. About 15,000 hospital beds will be needed out of approximately 440,000 existing beds. The main weak point is the severe shortage of medical personnel with experience in typical battlefield injuries – only a few hundred doctors in Germany have such experience. In March 2026, the Bundeswehr conducted the Medic Quadriga 2026 exercise, which simulated an attack on Lithuania and a full evacuation chain. Concurrently, the reactivation of a Cold War-era underground bunker is being considered, and the German government is advancing a new health security law for managing the medical system in emergencies. Professor Benedikt Friemert, the medical director of the military hospital in Ulm, stated that the system is not ready.
