A program that accompanies elderly people after hospitalization

Meuhedet Health Fund, together with the Ministry of Health and Joint-ESHEL, has launched the "Bridge to Health" program to support elderly people after hospital discharge. The program includes proactive patient identification, comprehensive assessment, and an individualized follow-up plan. Research showed a reduction in emergency hospitalizations among participants to 10.4% compared to 19% in the control group.

Meuhedet Health Fund, in collaboration with the Ministry of Health and the Joint-ESHEL organization, is implementing the "Bridge to Health" program aimed at supporting elderly people after hospital discharge. The program is based on the concept of proactive medicine: nurses themselves contact patients over 65, conduct a comprehensive assessment of physical and emotional state, fall risk, and ability to self-care. Based on the assessment, an individual plan is created, including interaction with a family doctor, clinic nurse, pharmacist, physiotherapist, and other specialists. About 950 people have already participated in the program. An accompanying study showed that the rate of emergency hospitalizations among participants was 10.4% compared to 19% in the control group, and 87% of participants remained stable or improved. Program leaders emphasize that the main goal is not only to prevent rehospitalization but also to help the patient maintain independence and confidence after returning home.
