Beneficiaries with complete data will be contacted directly by phone as soon as humanitarian shipments and aid become available in their areas. The form requires accurate entry of basic data, including personal data (full name, gender, date of birth), family data (ID numbers for husband and wife, names of family members), displacement and residential data (original governorate before displacement and current address), and demographic and health indicators (number of family members, number of males and females, number of elderly aged 60 and over, number of children under 6 years), in addition to clarifying special cases such as disabilities or chronic diseases to determine humanitarian need priorities.
